Greenlake Primary Care is currently accepting new patients into our our Membership programs. We offer Direct Primary care for all of your primary care needs ages birth to 64. Medicare beneficiaries may join our Concierge Membership program. We also offer Medical Management for Eating Disorders through our Premier care program. All of our programs include exclusive access to your provider via text, email or phone. Membership medicine allows for each provider to have smaller panel size and more time to focus on your needs. Personalized attention from your doctor results in healthier patients and a closer relationship with your provider.

- Your list of concerns
- Your current medications
- Contact information of previous provider
- Immunization history (for children)
