Your medical records are confidential. One of the nice things about DPC is that your insurance does NOT have the right to look at your medical records without your direct consent.  We also do not share records in “Care Everywhere”. When we place referrals that you request, you consent to having the specialist access to the notes we fax required by the referral. If you would prefer to self refer, let us know. Most referrals are not accepted without records. If you would like to release your medical records for any reason, to anyone, please sign this release form. It is submitted to us electronically so you do not need to print it.

Patients ask why our messaging system does not have full medical records. The nature of our interdisciplinary practice means that with a traditional portal, parents of adolescents and psychiatric patients would have full access to records without a release. This can be damaging, so we release records as requested individually. Usually your doctor will put the care plan from individual visits in Spruce.
